In 2015 Somerset Clinical Commissioning Group commissioned the Nursing Home Support Service, to support nursing homes with improving quality and reducing avoidable hospital admissions.
The Service is made up of Michelle Bell (registered nurse) and a Social Worker currently waiting to be appointed, we have access to specialists within the CCG such as our Infection Control Specialist Nurse, Medicines Management and the Safeguarding Team
The Service can support nursing homes with sign posting to resources relating to training and clinical support, interim training measures where appropriate relating to Tissue Viability, MCA, DOLs.
The Service will support with building and maintaining relationships with both of the Acute trusts in Somerset through the regular Learning Engagement meetings, (LEM) that are held 3 times a year at each, it will offer a forum for discussion and up to date evidence based knowledge and information training sessions.

The Service provides 6 weekly Newsletters providing up to date issue's, articles from specialist to support the Nursing home staff to optimise the care they provide. It also offers a full A4 side of resources of education available and up to date relevant information, these Links are displayed below.
The aim of our Service is to support Nursing homes where appropriate to care for Residents within their own home with the right support without unnecessary avoidable admission to hospital where this may not be the residents wishes or in their best interest.
The Service is also a means for Nursing home providers to be able to communicate potential inequalities within this care sector to enable the CCG to make changes and offer support where possible. The Individuals within the NHSS will vocalise concerns identified at both internal and external meetings and advocate on behalf of the Nursing homes, support requested and issue highlighted.
